Who it is for
People with support coordination in their NDIS (National Disability Insurance Scheme) plan. Specialist Support Coordination is for situations that are complex or carry a high level of risk.

How it is paid for
Through your NDIS plan. Support coordination needs to be included in your plan before we can start.

What we offer
Two kinds of support coordination.

Coordination of Supports
Our coordinators help you put your plan into action. They find providers, set up your services, and keep them working together as things change.

Specialist Support Coordination
For situations that are complex or carry a high level of risk. It is usually more intensive, and for a set period, while things are made stable and set up to work.
Our team
Who you will work with.

Social workers
For children and families, especially where NDIS supports sit alongside services from the NSW Department of Communities and Justice. Also for adults with complex psychosocial disability, where NDIS supports and NSW mental health services need to work together.

Registered nurses
For people who need high intensity supports, the NDIS term for daily care that needs specific clinical training, such as tube feeding, tracheostomy care or complex wound care.
Other services
Working alongside other services.
Some parts of your life are supported by the NDIS, and some by other services, like child protection or mental health teams. We do not replace those services. We help your NDIS supports and theirs fit together, so nothing falls through the gaps.
Your choice
Your choice, always.
together² also provides other supports, like Supported Independent Living and in-home support. Using our support coordination never means you have to use our other services. When we suggest a service, we will tell you if it is one of ours, give you other options (at least three where we can), and respect what you choose. Choosing another provider will never affect your support coordination.

Steven and the support coordination team take the first call, and will either get you started or pass you to the right person. Call any weekday, 9am to 5pm.
