Activity Planning

You tell us what activities are important to you and that’s what we’ll do. From movies, crafting or gym classes, to taking a holiday and escaping the daily grind, we design programs around your interests and needs – whatever they may be.

Alternative Day Programs

Our alternative Day Programs are focused on capacity building supports, we don’t have centres that are separate from the rest of the community, our centres are inclusive welcoming places that encourage the building of communities and friendship.

Short Breaks

What is life without a break?! Take the time-out you deserve while accessing the support you need to do it safely and independently. Our own short-term accommodation options designed just for this reason: On your own, with a friend or in a group – tell us what suits you best and we’ll make it happen.

24/7 Support

Whether you need support or company in the evenings or help over the weekend to enable you to get some shopping done, catch up with friends or attend an event, we are here for you 24/7.

Recreational Activities

Want to see a show or hit the gym? We work with local businesses, including gym’s and local theatre companies, to ensure that you have full access to all the activities that matter to you in your local community.

 

Accommodation Solutions

It’s not a house it’s your home – so where and how you live should suit you. Whatever your disability, we’ll help you find an accommodation solution that empowers your independence and fits your personality.

Supported Independent Living (SIL)

Because group homes don’t suit everyone! Supported Independent Living is about you as an individual and what works for you. We provide a range of SIL options including single and double units. There will be no staff offices in your home, just clusters of SIL housing with shared supports to maximise your independence and privacy.

Individualised Living Options (ILO)

If you are looking for a more personalised independent living option, we can help you explore and design the home and supports that you want. Maybe you need help with managing your own staff or you want us to handle everything, from finding accomodation to managing your staff, for you. You choose how you want to be supported and we’ll make it happen.

Plan Management & Support Coordination

When you have our team in your corner, we don’t just look after your admin and take the paperwork off your hands, we give you the support, info and tools to confidently manage your own funds and get the supports you need.

Specialist Support Coordination

Need an extra level of support? From occupational therapy, psychology or social workers, our specialist support coordination helps reduce any barriers to implementing or using your NDIS plan. And yes, it includes full management of your plan as well!

We will help you to achieve clear goals for the duration of your first plan along with support from our qualified social workers to navigate the complexities of health and community justice services as needed. Our aim is to decrease the need for this high-intensity support to continue – if you no longer need this, that means we have done our job!

Individual Support

Daily support. Weekly support. ‘Sometimes’ support.

Our dedicated support worker will work with you one-on-one to develop the skills you need to live independently in your own home and become an active community member through involvement in leisure, recreation, sport, education and employment.

Working in partnership with you and the people in your life who know you best, you’ll be empowered to realise your potential and live life your way.

Ageing Disability Service

Ageing with a disability sometimes requires extra care and attention to maintain your quality of life. Our team is experienced in both aged and disability care and will support you with accessing the care services you need while keeping your independence.

Residential Placements for Children and Occasional Respite

Together² is an approved voluntary out-of-home (VOOHC) care provider registered by the Office of the Children’s Guardian to provide centre-based out-of-home care. This means that if support is needed for children and young people with disability when outside the family home, we can help.

We provide respite when, as a family member or carer, you need assistance for things like:

  • Overnight support
  • Transport; eg. collecting the child from school
  • Community activities such as sport
  • After school care/during school holidays

In-Home Care

Our new wellbeing teams are a radical new model for delivering care and support within our community to create true wellbeing. How? By ensuring people are safe and well, are supported to do more of what matters to them and can stay connected to others and their community.

Our teams are:

  • self-managed and small neighbourhood teams
  • values-led at every step
  • focussed on co-design, supporting people to make decisions about their own life and support
  • committed to the wellbeing of the people they support and the team members
